Job Title: Occupational Therapist - Home Health Care Specialist

Job Summary: Centra Healthcare Allied is seeking a skilled Occupational Therapist to join our team as a Home Health Care Specialist. As a key member of our team, you will be responsible for providing high-quality occupational therapy services to patients in their homes.

Responsibilities:

  • Provide occupational therapy services to patients in their homes, focusing on improving their functional abilities and independence.
  • Conduct thorough assessments of patients' needs and develop personalized treatment plans to address their goals and objectives.
  • Implement and evaluate the effectiveness of treatment plans, making adjustments as needed to ensure optimal patient outcomes.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
  • Document patient progress and maintain accurate records of treatment plans, progress notes, and outcomes.

Requirements:

  • 1 year of recent experience as an Occupational Therapist specializing in Home Health Care.
  • Current Occupational Therapist license within the state of practice.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
