Job Overview

Position Summary:
Under the supervision of the MDS Coordinator, the MDS Coordinator Assistant plays a crucial role in ensuring the timely and precise completion of Federal and State assessment tools.

Key Responsibilities:
This role necessitates a comprehensive understanding of MDS regulations and guidelines as outlined in the MDS User's Manual, which includes item coding, RAPS, CAA's, Care Planning, electronic submission, scheduling, and the PPS process. Knowledge of Medicare regulations, including coverage, benefit periods, and certification, is essential.

Essential Duties:

  • Assist in maintaining MDS and Medicare Tracking Tools.
  • Support skilled services decision-making and initiate Medicare non-coverage letters and expedited determination notices as directed.
  • Review health records for each resident, noting significant changes in:
    • Activity levels
    • Eating habits
    • Physical, mental, and emotional well-being
  • Assist in planning and adjusting Medicare assessment schedules.
  • Coordinate with the interdisciplinary team (IDT) to ensure timely completion of assignments, Care Plans, and CAA summaries.
  • Provide guidance on the Care Planning process to nursing staff and team members, including ADL training.
  • Complete the Medicare Daily Documentation Guide upon admission.
  • Participate in studies aimed at enhancing nursing care quality.
  • Gather baseline information to identify potential and acute issues, ensuring thorough completion of MDS nursing admission assessments and quarterly evaluations.
  • Prepare for and attend Care Plan conferences with teams and families in a professional manner.
  • Monitor and assess residents' overall health, focusing on body systems, skin integrity, weight, and hydration status, reporting any changes promptly.

Standard Requirements:

  • Maintain resident rights and foster an environment that promotes privacy, dignity, and well-being.
  • Adhere to safety protocols, including OSHA standards and disaster plans.
  • Ensure confidentiality of all data, including resident and operational information.
  • Comply with HIPAA regulations and Quality Assurance requirements.
  • Engage in teamwork, cooperating with colleagues and maintaining effective communication.
  • Report any incidents of resident abuse or neglect to the appropriate authorities.
  • Stay informed on age-specific developmental factors relevant to adult and geriatric care.
  • Participate in ongoing education to maintain current practice standards.

Qualifications:

  • Graduate of an accredited nursing program; current nursing license as an RN or LPN.
  • Knowledge of state-specific MCD Case Mix and coding guidelines.
  • Current CPR certification is required.
  • Ability to perform physical tasks, including lifting and transferring residents.
  • Proficient in reading, writing, and communication skills.
  • Minimum of one year of experience in related nursing roles is preferred.
