Key Responsibilities- Conduct thorough assessments of patients to identify their unique care needs and develop personalized care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
- Monitor patient progress, making adjustments to care plans as needed to ensure optimal health outcomes.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Associate's or Bachelor's degree in Nursing.
- Current RN licensure in the state of practice.
- At least one year of recent acute care experience in an institutional setting, with a preference for home health care experience.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
