Job Summary:

We are seeking a skilled Speech Language Pathologist to join our team at Frontier Management (OR) as a Per Diem Speech Language Pathologist. As a key member of our rehabilitation team, you will be responsible for evaluating residents and developing treatment plans to meet their unique needs.

Responsibilities:

  • Evaluate residents and develop a comprehensive treatment plan that addresses their individual needs and goals.
  • Conduct assessments and develop strategies to help residents achieve independence in their daily activities.
  • Monitor and record resident progress, providing regular updates to the healthcare team.
  • Collaborate with other healthcare professionals to ensure seamless care and support for residents.
  • Develop and implement individualized plans to help residents overcome communication and swallowing disorders.
  • Maintain accurate and detailed records of resident care and treatment.
  • Participate in ongoing education and training to stay current with best practices in speech-language pathology.

Requirements:

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current licensure as a Speech-Language Pathologist in the state of New York.
  • Experience working in a rehabilitation setting, preferably in a long-term care facility.
