In this role, you will be responsible for evaluating and assessing patient needs, developing and implementing comprehensive care plans, and collaborating with our interdisciplinary care team to ensure seamless delivery of services. Salary Information: $80,000 - $110,000 per year, depending on experience.

Key Responsibilities:

  • Evaluate and assess patient needs to determine the most effective course of treatment
  • Develop and implement personalized care plans that address the unique needs of each patient
  • Collaborate with our interdisciplinary care team to ensure seamless delivery of services
  • Communicate effectively with patients, families, and healthcare professionals to coordinate care and ensure positive outcomes
  • Document patient progress and outcomes in accordance with regulatory requirements

Requirements:

  • Current Occupational Therapist License, valid state driver's license, and auto liability insurance
  • One year of experience as an Occupational Therapist in an acute care, rehabilitation, or home health setting preferred
