Job Summary

Key Responsibilities
  • Coordinate the completion of service agreements and service authorizations.
  • Work with families to develop Employer of Record Implementation Strategies.
  • Maintain accurate and concise case records and progress notes on each member.
  • Complete other reports and forms as assigned.
  • Seek out referral sources in the community and establish linkages when appropriate.
  • Collaborate with other staff and programs regarding the needs of shared members.
  • Accompany members to referral services and act as an advocate on their behalf.
Other Responsibilities
  • Assist the supervisor in special projects.
  • Meet weekly with the supervisor to discuss cases.
  • Maintain an accurate work schedule and mileage records.
  • Perform other related responsibilities as assigned.
Requirements

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required.

Qualifications
  • Bachelor's degree in a human or social service field.
  • At least one year of direct experience working with adults with developmental disabilities.
  • Must be QIDP eligible upon hire.
  • Must have a valid driver's license.
  • Must have access to a reliable vehicle for travel throughout the Chicagoland area.
  • Must be willing to travel up to 70% of the time, with the remaining 30% spent working from the Rose Center office.
Work Environment

The work environment is typically low to moderate noise level. Employees may work alone or with others, and may have verbal and face-to-face contact with colleagues and clients.
