Risk Assessment Provider Opportunity

We are seeking a skilled Nurse Practitioner to join our team as a Risk Assessment Provider in Maine. As a Risk Assessment Provider, you will be responsible for conducting health risk assessments on insured plan patients in their homes or designated areas.

Key Responsibilities:
  • Conduct thorough health risk assessments to identify active and chronic disease conditions, as well as physical, mental, and social needs.
  • Obtain vital signs, measure BMI, review pharmacological therapy, and conduct physical examinations.
  • Perform necessary screenings to capture HEDIS measures and close gaps in care.
  • Educate and counsel patients and families on identified conditions.

Requirements:
  • Active Nurse Practitioner state license and board certifications.
  • At least 1 year of experience preferred.
  • Risk Assessment coding experience/knowledge preferred.
  • Valid state driver's license and automobile insurance.
  • Ability to travel within selected counties.
  • Strong computer skills, including Word, Excel, EMRs, and web-based applications.
