Job Summary

Agrace HospiceCare is seeking a skilled Registered Nurse to join our Resource Team. As a key member of our interdisciplinary team, you will provide comprehensive nursing care to patients and families, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities
  • Provide patient and family assessments, develop and implement plans of care, and evaluate patient outcomes.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Respond to patient and family needs, changes in condition, and new patient admissions.
  • Provide interventions, treatments, and pain management to meet patient needs.
  • Document interventions, outcomes, and changes in patient condition in the electronic health record.
Requirements
  • ADN or BSN required.
  • Current, unrestricted Registered Nursing License in the state of employment.
  • One-year nursing experience preferred.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life care.
  • Ability to maintain professional boundaries with patients, families, and co-workers.
