Role Summary

The Licensed Practical Nurse plays a crucial role in delivering nursing care, support, and treatment to children and adolescents, adhering to the guidelines set forth by InCommunity and the relevant nursing authorities. The LPN will provide services in a short-term crisis environment tailored for children diagnosed with autism spectrum disorders, as well as those with intellectual and developmental disabilities accompanied by psychiatric symptoms. Collaboration with an interdisciplinary team, including behavior analysts, psychiatrists, and psychologists, is essential to ensure comprehensive care.

Key Responsibilities

  • Lead the staff in the crisis home environment.
  • Oversee the management and operations of the unit.
  • Maintain cleanliness and ensure all equipment is functioning properly.
  • Coordinate individualized nursing care.
  • Communicate with medical professionals as necessary regarding patient conditions.
  • Complete assigned shifts and ensure proper handover with incoming nursing staff.
  • Deliver skilled nursing care and conduct health assessments as required.
  • Administer medications and supervise their administration.
  • Ensure adherence to the daily activity schedule, promoting structured skills training and educational activities.
  • Manage medication protocols effectively.
  • Oversee daily unit operations during assigned shifts.
  • Report any changes in patient conditions to the Registered Nurse and family promptly.
  • Maintain a safe and productive environment for all clients.
  • Address and resolve issues as they arise to meet individual needs effectively.
  • Coordinate with pharmacies for medication orders as needed.
  • Monitor medication administration and assist staff with related concerns.
  • Participate in staff meetings to discuss operational matters.
  • Engage in routine consultations with the Registered Nurse to address ongoing issues.
  • Assist in the recruitment, training, and evaluation of crisis home staff.
  • Conduct performance reviews and manage disciplinary actions as necessary.
  • Review documentation from direct care staff to ensure accuracy and compliance.
  • Maintain organized and systematic recordkeeping practices.
  • Provide guidance on crisis intervention and support for critical incidents.
  • Complete incident reports as required.
  • Manage house funds responsibly for recreational activities.
  • Collaborate with various team members to ensure efficient home management in compliance with regulations.
  • Engage in professional development activities such as training and conferences.
  • Accept additional responsibilities as assigned.

Qualifications

To be successful in this role, candidates must demonstrate the ability to fulfill essential job functions effectively. Flexibility in scheduling is required to provide necessary services and support, including on-call availability. Physical stamina is essential to manage periods of high activity and stress, with the ability to lift 50 pounds or more. Candidates should be prepared to travel independently in various locations.

Education and Experience

  • Graduation from an accredited School of Nursing.
  • A minimum of two years of experience working with individuals with developmental disabilities.
  • At least three years of recent experience with individuals diagnosed with autism spectrum disorders and their families.
