This role will be based at Sentara Careplex Hospital and Sentara Williamsburg Regional Medical Center in Virginia. The Advanced Practice Provider (APP) will play a crucial role in delivering clinical care, guidance, and education to patients within the palliative care spectrum as part of a collaborative healthcare team.
Key Responsibilities:
- Work on a PRN/Flex basis
- Schedule shifts through mutual agreement, typically 8-hour shifts from 7 AM to 5 PM
- Conduct palliative care consultations and follow-up visits for hospitalized patients facing acute palliative care challenges, including symptom management and intricate medical decision-making
- Lead family discussions regarding ongoing medical decisions and advance care planning
- Collaborate in formulating suitable discharge plans that align with established care objectives, ensuring seamless cooperation with Hospice services to maintain continuity of care.
