Job Title: Perm Speech Language Pathologist

HexaQuEST Health, Inc. is seeking a skilled Perm Speech Language Pathologist to join our team in a SNF setting. As a key member of our rehabilitation team, you will be responsible for providing high-quality speech therapy services to patients with long-term care and dementia care needs.

Key Responsibilities:
  • Develop and implement individualized speech therapy plans for patients with long-term care and dementia care needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients.
  • Utilize electronic documentation via Net Health to track patient progress and communicate with team members.
  • Work in a fast-paced SNF environment with a focus on delivering exceptional patient care.
  • Participate in ongoing education and training to stay up-to-date on best practices in speech therapy.
Requirements:
  • Master's degree in Speech-Language Pathology or related field.
  • Current licensure as a Speech-Language Pathologist in the state of practice.
  • Experience working in a SNF setting with long-term care and dementia care populations.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
