Job SummaryWe are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with Pinnacle Home Care Inc., you will play a vital role in providing patient-centered care to our clients, working closely with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Provide Skilled Nursing Care: Under the supervision of a Registered Nurse, you will perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care.
- Assist with Patient Care Planning: Assist with the preparation, implementation, and continuing evaluation of the client care plan, ensuring that patient needs are met and goals are achieved.
- Communicate Effectively: Foster cooperative effort among personnel by understanding the functions of other persons involved in client care and by active participation in team and staff conferences.
- Document Patient Information: Prepare clinical and progress notes, including interim orders written with measurable goals and time frames, and the integration of these orders into the patient's plan of care under the supervision of a Registered Nurse.
- Monitor Patient Condition: Observe, record, and report to the appropriate person patient symptoms, reactions, and changes, including signs and symptoms which may be indicative of untoward changes.
- Participate in Quality Improvement: Actively participate in nursing organizations and promote and participate in in-service programs, ensuring that patient care is delivered in accordance with the Agency's QAPI program.
- IV Certification: Must be certified in IV therapy.
- LPN License: Must be a licensed practical nurse in good standing.
