Role Overview:
The Hospice Clinical Manager is responsible for overseeing the case management of all patient services. This role includes ensuring accurate documentation for both patient care and staff performance, supervising assigned care staff, and assisting the Administrator as needed.

Key Responsibilities:

  • Coordinate comprehensive case management for all patient services.
  • Ensure proper documentation for patient care and staff performance evaluations.
  • Supervise and support assigned care staff.
  • Assist the Administrator with various operational tasks.
  • Provide direct patient care as needed, including case management, Oasis care planning, and staff assignments.
  • Optimize the use of existing staffing resources for effective service delivery.

Qualifications:

  • Active RN licensure in North Carolina, with completion of nursing education from an accredited institution.
  • A minimum of three (3) years of clinical nursing experience across diverse healthcare settings, with a preference for hospice experience.
  • Thorough understanding of hospice and homecare regulations.
  • Proven ability to independently manage workload, deliver cost-effective hospice services, and maintain timely and accurate documentation.
  • Strong critical thinking and communication skills, both verbal and written.
  • Proficient in using personal computers and familiar with various software applications.
  • Current CPR certification.
