Job Summary:

We are seeking a skilled Patient Care Coordinator to join our team in the specialty clinic setting. As a key member of our healthcare team, you will provide direct patient care, education, and support to patients while maintaining advanced knowledge of diagnoses, medications, and treatment options specific to the specialty.

Main Responsibilities:

  • ">
  • Provide direct patient care in an outpatient specialty setting, evaluating effectiveness of care with physicians.">
  • Serve as primary contact for patient/family members, performing phone triage effectively and involving physicians when needed.">
  • Triage current patient population and incoming referrals, prioritizing scheduling of office visits and testing.">
  • Assist with in-office procedures, providing comfort and safety measures for patients before, during, and after the procedure.">
  • Identify learning needs of patients/family members, developing educational tools to promote compliance with treatment plans.">
  • Maintain a safe environment, completing environmental rounds and enforcing patient care standards.">
  • Champion the development and implementation of orientation programs, ensuring new staff are educated and prepared to give safe, effective care.">
  • Work with team members to plan, develop, and implement educational programs and seminars in the community and within Prisma Health.">
  • Evaluate own nursing practice, maintaining competency through education and professional activities.">

Requirements:

  • ">
  • Associate's Degree in Nursing">
  • 1 year Clinical Nursing Experience within specialty (or 1 year LPN experience)">
  • Certification in specialty area (preferred)">

Estimated Salary: $60,000 - $80,000 per year, depending on location and experience.
