Job Title: Home Health LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care in the comfort of their own homes.

Job Summary:

The Home Health LPN is responsible for delivering personalized care to patients with diverse needs, working collaboratively with the healthcare team to develop and implement effective care plans. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, documenting findings and developing care plans.
  • Collaborate with the healthcare team to coordinate care, including referrals to specialists and other healthcare services.
  • Educate patients and their families on disease management, medication use, and self-care techniques.
  • Maintain accurate and timely documentation of patient interactions, including progress notes and care plans.
Requirements:
  • Current LPN licensure in the state of practice.
  • Valid driver's license and reliable transportation.
  • Basic Life Support (BLS) certification.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
