Home Health Licensed Practical Nurse (LPN) Job Description

At Interim HealthCare of Lexington MA, we're seeking a skilled Home Health Licensed Practical Nurse (LPN) to join our team. As a Home Health LPN, you'll have the opportunity to provide high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities:
  • Provide home-based care to patients with illnesses, injuries, and chronic diseases, working under the supervision of an RN.
  • Collaborate with a team of physicians, RNs, CNAs, and aides to deliver quality patient care.
  • Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
  • Assist the RN in carrying out patients' plans of care and teaching patient self-care techniques.
  • Educate family members on patients' plans of care, medication, and home safety.
  • Work with the interdisciplinary team to coordinate patient care and plan for discharge.
Requirements:
  • Graduate of an accredited practical nursing program and active LPN license in Massachusetts.
  • Minimum of 1 year of LPN experience, ideally in home healthcare, or completion of our office's preceptor program.
  • CPR certification.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
