About the Role:

As an RN Case Manager at Hospice of Green Country, you will have the opportunity to make a meaningful difference in the lives of patients and their families. You will be responsible for coordinating care, facilitating patient/family care conferences, and educating patients and families about the disease process and care administration.

**Responsibilities:**
• Provide direct and indirect nursing care for assigned patients.
• Collaborate with healthcare teams to develop and implement individualized care plans.
• Educate patients and families on disease management and treatment options.
• Participate in quality improvement initiatives to ensure optimal patient outcomes.

**Requirements:**
• Current RN licensure in Oklahoma.
• Bachelor degree in Nursing preferred.
• 1 year experience in a healthcare setting, preferably in hospice or home health care.

**Compensation:** $62,500 - $72,500 annually, based on experience and qualifications.
