Cancer Patient Care Coordinator

We are seeking a highly skilled Cancer Patient Care Coordinator to join our team at Oklahoma Cancer Specialists Management Company. As a key member of our oncology team, you will be responsible for providing comprehensive care coordination services to patients and their families.

Responsibilities:
- To work collaboratively with RN Case Managers and other healthcare teams to ensure seamless case coordination and effective communication among all stakeholders.
- To conduct thorough psychosocial assessments and develop personalized intervention plans to address the unique needs of each patient.
- To coordinate the delivery of care through various oncology teams, including physicians, Nurse Navigators, and other support staff.
- To provide education and support to patients and families on medical recommendations and treatment options.
Requirements:
- Bachelor's degree in Social Work, Social Sciences, Human Services, or related field. Master's degree preferred.
- Licensed Clinical Social Worker (LCSW) or LCSW eligibility required.
- At least one year of previous medical social work experience, preferably in oncology.
- Demonstrated ability to work effectively in a team environment and communicate complex information to patients and families.
