Position Overview:

  • Input evaluation data into electronic systems and ensure timely transmission as needed.
  • Document any concerns or discrepancies encountered during the evaluation process.
  • Facilitate continuity of care to minimize treatment gaps and enhance patient outcomes.
  • Discuss available healthcare options and support services with patients and their families.
  • Coordinate communication between the Call Center and eligible patients for managed care enrollment.
  • Strengthen collaboration with interdisciplinary care teams to improve overall care coordination.
  • Gather quality review data and necessary documentation for performance measurement.
  • Maintain an up-to-date understanding of community resources and payor requirements.
  • Adhere to established performance standards and criteria.
  • Perform additional duties as assigned by management.

Qualifications:

  • Minimum of two years of experience in a home care setting within the last decade.
  • Recent experience in conducting thorough patient assessments.
  • Current Registered Nurse Licensure in New York State.

Working Hours:

  • Full-time positions require a commitment of four days per week with specified start times.
  • Part-time roles necessitate a minimum of five assessments weekly on flexible days.
  • Initial training spans 13 days with specified hours.
  • Post-training, availability for shadowing and precepting is required during weekdays.
