Speech Language Pathologist Job Description

We are seeking a highly skilled Speech Language Pathologist to join our team at HealthPRO Heritage. As a Speech Language Pathologist, you will play a vital role in helping patients overcome communication disorders and achieve their maximum potential.

Responsibilities:
  • Evaluate patients to determine the impact of available resident information on the assessment and treatment process.
  • Develop and implement individualized treatment programs based on initial evaluations and containing achievable functional goals.
  • Comply with evaluation, treatment, and documentation guidelines, and maintain timely and accurate departmental records.
  • Enter progress notes in the resident record following each treatment session and revise treatment programs as needed.
  • Orient residents to program services and treatment procedures, inform them of potential risks, and instruct them on therapeutic procedures to be continued at home.
  • Work with nursing staff, residents, and families to maximize residents' use of functional abilities and assist in the referral process when additional services are required.
  • Attend and participate in rehabilitation team conferences, department meetings, and quality improvement activities.
Qualifications:
  • Masters Degree in Speech and Language Therapy.
  • Current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) credential offered by the American Speech-Language-Hearing Association.
  • Valid state Speech Language Pathology licensure.
