Job Summary:We are seeking a highly skilled and compassionate RN Care Manager to develop, assess, and facilitate complex care management activities for our members. As a RN Care Manager, you will provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.
Key Responsibilities:- Evaluate member needs, barriers to accessing care, and social determinants of health to identify priority areas for intervention.
- Develop ongoing care plans and collaborate with providers to identify resources and specialists to address member needs.
- Identify problems and provide care management interventions to ensure members receive adequate and appropriate care.
- Coordinate with members, caregivers, and providers to ensure timely access to needed care and services.
- Provide education and support to members and caregivers on disease processes, care options, and healthcare benefits.
- Collect and document member information and care management activities to ensure compliance with regulatory requirements.
- Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2-4 years of related experience.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required.

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law. Total compensation may also include additional forms of incentives.
