Key Responsibilities- Comprehensive Clinical Assessments: Conduct thorough clinical assessments using an organization-approved format and diagnose within your scope of practice and credentials.
- Treatment Planning: Develop treatment plans using an organization-approved format in coordination with patients, caregivers, and treatment teams.
- Interventions: Coordinate and implement individual, group, and family interventions using evidence-based practices.
- Documentation: Maintain timely documentation of patient care and contacts, adhering to NC DHHS Service Definition documentation standards and organization policies.
- Team Collaboration: Contribute to the development of a successful therapeutic treatment program in a respectful, professional, and culturally sensitive manner.
- Client Rights: Comply with NC DHHS Clients Rights Rules and applicable organization policies.
- Professional Development: Successfully complete required trainings and maintain licensure and certification requirements.
- Team Participation: Participate in case reviews during PCHH huddles and assist patients in developing Behavioral Health self-management goals and care plans.
- Licensed Behavioral Health Professional working in an outpatient medical setting.
- Ability to conduct comprehensive clinical assessments and diagnose within scope of practice and credentials.
