Compensation Package
The estimated annual salary for this role is $85,000-$90,000, depending on location, job-related knowledge, skills, and experience. Additionally, Amedisys provides a comprehensive benefits package, including choice of affordable PPO or HSA medical plans, paid time off, up to $1,300 in free healthcare services per year, and more.

Responsibilities
As a Home Health Hospice Case Manager, your key responsibilities will include:
• Performing patient assessments and collaborating with the care team to develop and implement a plan of care.
• Making referrals to other hospice care disciplines as needed or documenting rationale for not doing so.
• Developing, preparing, and maintaining individualized patient care progress records with accuracy, timeliness, and according to care center policies.
• Recording pain/symptom management changes/outcomes as appropriate.
• Teaching patients, families, and caregivers appropriate patient care techniques.
• Regularly communicating with clinical manager and care team.
• Performing on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Requirements
To be successful in this role, you will need:
• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
