Job Title: Case Manager - Veteran Support Specialist

Job Summary: We are seeking a skilled Case Manager to join our team at WestCare Foundation. In this role, you will work collaboratively with community resources to provide essential services to veterans and their families.

Key Responsibilities:

* Assess the resource needs of participants and develop personalized plans to address their challenges
* Educate and refer participants to community agencies and businesses that cater to their needs
* Assist veterans in establishing or re-establishing support systems within their community
* Act as an advocate on behalf of the veterans to improve access to needed services, with a focus on VA
* Participate in staffing decisions regarding the veterans' current strengths and resources
* Complete electronic documentation of services in a timely manner

Requirements:

* A minimum of two years of experience in assisting veterans and families in transitioning from military to civilian life
* High School Diploma or equivalent
* Ability to exercise good judgment and discretion
* Ability to operate a computer and complete documents in Microsoft Word program formats
* Ability to respond as needed to crisis situations in an efficient manner
* Ability to work well in a team environment
* Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality
* Professional appearance and demeanor
* Must be culturally/linguistically sensitive to populations served
* Military/Veteran preferred
* Must be able to successfully obtain and maintain a criminal background clearance
* Valid drivers license and ability to obtain and maintain WestCare's vehicle insurance
* CPR Certification, First Aid Certification, and an annual Tuberculosis test

Working Conditions:

Work is primarily performed in a residential setting.

Work varied shifts, including weekends and holidays.
