Job Summary
We are seeking a highly skilled and compassionate Patient Services Coordinator, Home Health to join our team. As a Patient Services Coordinator, you will play a critical role in linking patients to the home health care they need, ensuring seamless coordination and communication between patients, families, and care teams.
Key Responsibilities
- Schedule patient services and coordinate with patients, families, and care team members to ensure timely and effective care delivery.
- Ensure that all agency field personnel are properly scheduled to achieve expected levels of productivity and provide care in the most efficient manner possible.
- Provide field caregivers with the necessary information to deliver high-quality patient care.
- Manage competing priorities and multifaceted challenges with ease.
Requirements
- Current Vocational/Practical Nurse License
- Valid state driver's license and auto liability insurance
- Home health experience preferred
