Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families. You will work collaboratively with our team of clinicians, case managers, and social workers to ensure seamless care coordination.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment in the patient's home or other community environments
  • Develop and implement individualized care plans to address patient needs and goals
  • Provide education to patients and their families on health management, disease prevention, and wellness
  • Collaborate with primary care physicians, case managers, and social workers to ensure comprehensive care coordination
  • Document patient information and care plans in accordance with Matrix Medical Network's guidelines and protocols
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, including prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record software
  • Excellent verbal and written communication skills
