About the Role:
The Home Care Licensed Practical Nurse provides direct nursing services to patients and families in their homes and communities. As part of our team, you will work collaboratively with patients, families, and healthcare professionals to deliver exceptional care and support.
Responsibilities:
- Provide skilled nursing services to patients in their homes and communities.
- Develop and implement individualized care plans to meet patient needs.
- Monitor patient progress, adjust care plans as needed, and communicate effectively with patients and families.
- Collaborate with healthcare teams to ensure seamless transitions of care.
Requirements:
- Graduate of an approved practical/vocational nursing program.
- Minimum two years of medical/surgical experience required.
- Current MA license to practice as practical/vocational Nurse.
- BLS healthcare provider certification.
Estimated Annual Salary: $70,000 - $90,000 per year, based on experience.
