Job Summary

We are seeking a skilled and compassionate Community Health Nurse (LPN/RN) to join our team at University House at Issaquah. As a key member of our healthcare team, you will be responsible for providing high-quality care to our residents, promoting their health and well-being, and collaborating with our Community Health Director to develop and implement effective health care services.

Responsibilities
  • Assessment and Care Planning: Conduct thorough assessments of residents to identify their health needs and develop personalized care plans to meet those needs.
  • Health Promotion and Education: Provide education and support to residents and their families on healthy living, disease prevention, and management of chronic conditions.
  • Collaboration and Communication: Work closely with our Community Health Director, other healthcare professionals, and residents to ensure seamless care coordination and effective communication.
  • Documentation and Reporting: Maintain accurate and up-to-date records of resident care, including progress notes, medication lists, and other relevant information.
Requirements
  • Education and Experience: Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Washington. Minimum 2 years of experience working with older adults, preferably in an assisted living facility.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work effectively in a team environment, and strong problem-solving and critical thinking skills.
  • Certifications and Training: Current CPR certification and ability to pass a DSHS background check.
