About the Role

The Housing Support Specialist will play a vital role in ensuring the smooth transition of residents from independent living to higher levels of care. This individual will be responsible for establishing relationships with community resources, providing education on available services, and assisting residents with benefit applications.

Key Responsibilities
  • Maintain effective communication with residents, families, and community partners.
  • Collaborate with local, state, and federal agencies to provide holistic care.
  • Develop and maintain records related to resident interventions and service referrals.
  • Provide general case management and monitor ongoing services.
  • Assist residents in completing health care documents.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in social work or a related field.
  • 2-3 years of experience working with the elderly or in a continuum of care setting.
  • Knowledge of HUD guidelines and regulations.
