Alluvion Health is seeking a highly skilled Licensed Clinical Behavioral Health Provider-Therapeutic Play to join our team. As a core member of the Patient Centered Medical Home/Collaborative Care Team, you will provide comprehensive patient-centered health care to children aged 2-12.

Key Responsibilities
  1. Deliver evidence-based therapeutic play interventions to children aged 2-12.
  2. Closely coordinate mental health care with the patient's primary care provider and other mental health providers.
  3. Evaluate and gather information from patients regarding mental health and substance abuse disorders.
  4. Use measurement-based tools to track patients' clinical symptoms and gather information about treatment side effects or complications.
  5. Coordinate with other members of the care team as needed.
  6. Support psychotropic medication management prescribed by PCPs.
  7. Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments.
  8. Participate in regularly scheduled caseload consultation with the Psychiatric Consultant.
  9. Facilitate patient engagement and follow-up in care.
  10. Evaluate patient care plans with care team.
  11. Document patient encounters, patient progress, treatment, and follow-up care in the electronic medical record system.
  12. Facilitate referrals for clinically indicated services outside Alluvion Health.
  13. Attend and participate in meetings and Quality Improvement activities as required.
  14. Serve as a member of committees as requested.
  15. Participate in peer review as part of Quality Improvement activities.
  16. Maintain confidentiality according to HIPAA.
  17. Work as a team player with staff and other community organizations.
Requirements
  • Masters in Social Work or Clinical Mental Health Counseling required.
  • Licensed Clinical Social Worker or Licensed Clinical Professional Counselor in the state of Montana.
  • Training in play therapy interventions.
  • Valid Driver's License issued by the State of Montana.
  • Intermediate to advanced software knowledge in Excel, Access, Word, Outlook, and the ability to learn and adequately operate Alluvion Health software applications.
  • Fluent in the English language.
Working Conditions
  • Work indoors in a climate-controlled environment 95% of the time.
  • OSHA Exposure Category #1.
Work Hours
  • Full-time, Exempt. Typically, a 40-hour workweek.
