Alluvion Health is seeking a highly skilled Licensed Clinical Behavioral Health Provider-Therapeutic Play to join our team. As a core member of the Patient Centered Medical Home/Collaborative Care Team, you will provide comprehensive patient-centered health care to children aged 2-12.
Key Responsibilities- Deliver evidence-based therapeutic play interventions to children aged 2-12.
- Closely coordinate mental health care with the patient's primary care provider and other mental health providers.
- Evaluate and gather information from patients regarding mental health and substance abuse disorders.
- Use measurement-based tools to track patients' clinical symptoms and gather information about treatment side effects or complications.
- Coordinate with other members of the care team as needed.
- Support psychotropic medication management prescribed by PCPs.
- Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments.
- Participate in regularly scheduled caseload consultation with the Psychiatric Consultant.
- Facilitate patient engagement and follow-up in care.
- Evaluate patient care plans with care team.
- Document patient encounters, patient progress, treatment, and follow-up care in the electronic medical record system.
- Facilitate referrals for clinically indicated services outside Alluvion Health.
- Attend and participate in meetings and Quality Improvement activities as required.
- Serve as a member of committees as requested.
- Participate in peer review as part of Quality Improvement activities.
- Maintain confidentiality according to HIPAA.
- Work as a team player with staff and other community organizations.
- Masters in Social Work or Clinical Mental Health Counseling required.
- Licensed Clinical Social Worker or Licensed Clinical Professional Counselor in the state of Montana.
- Training in play therapy interventions.
- Valid Driver's License issued by the State of Montana.
- Intermediate to advanced software knowledge in Excel, Access, Word, Outlook, and the ability to learn and adequately operate Alluvion Health software applications.
- Fluent in the English language.
- Work indoors in a climate-controlled environment 95% of the time.
- OSHA Exposure Category #1.
- Full-time, Exempt. Typically, a 40-hour workweek.
