Care Coordinator RN Supervisor Position

We are seeking a skilled and experienced RN Case Manager Supervisor to join our team at Professional Case Management. This role offers the opportunity to oversee and provide direct care to clients, while managing a team of RN Case Managers.

Job Responsibilities:

  • Supervise and manage a team of RN Case Managers, ensuring that they have the necessary resources and support to deliver high-quality patient care.
  • Conduct regular performance evaluations and provide feedback to team members, helping them to develop their skills and improve their practice.
  • Collaborate with other healthcare professionals, such as physicians and social workers, to ensure that patients receive comprehensive and coordinated care.
  • Develop and implement effective care plans, taking into account the unique needs and goals of each patient.
  • Monitor and evaluate the quality of care provided by team members, making adjustments as needed to ensure that patients receive the best possible outcomes.

Requirements:

  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • BSN degree required; MSN or higher preferred
