Job Summary

Responsibilities
  • Provide comprehensive home healthcare services to clients, including wound care, medication management, and education on healthy living practices.
  • Develop and implement personalized care plans that cater to each client's unique needs and goals.
  • Collaborate with our interdisciplinary team to ensure seamless coordination of care and effective communication with clients and their families.
  • Maintain accurate and up-to-date records of client interactions, treatments, and outcomes.
Requirements
  • Current, unrestricted license as an LPN/LVN in the state(s) of practice.
  • Minimum one year experience as an LPN/LVN in an acute care setting or equivalent; new graduate nurses considered.
  • Current CPR certification and TB test.
  • Possess strong communication and interpersonal skills, with a commitment to patient safety and respect for individual, cultural, and spiritual differences.
