Clinical Care Coordinator Nurse

We are seeking a skilled Clinical Care Coordinator Nurse to join our team at the US Department of Health and Human Services. As a Clinical Care Coordinator Nurse, you will provide comprehensive nursing care to patients across the age spectrum, implementing and initiating standardized disease management and health prevention guidelines across the patient care continuum.
Key Responsibilities- Manage and coordinate patient care and resources for a caseload of patients across the continuum of care and through care transitions.
- Develop and maintain patient registries based on disease, health condition, and/or health prevention initiatives.
- Provide patient and family education pertinent to a wide range of conditions and health factors.
- Assist with triage of patients, including triaging telephone calls and walk-in patients.
- Prepare patients for tests, exams, and treatments.
To qualify for this position, you must have a graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program. You must also possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, you must have at least one (1) full year of professional nursing experience (equivalent to the GS-9 grade level) in the management of nursing care in outpatient primary care environments and/or case management, emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor, and evaluate health care options and services which are consistent with patient-centered medical home principles.
