Key Responsibilities- Provide Physical Therapy treatment to clients in their homes
- Assess client needs and develop personalized treatment plans
- Educate clients and their families on treatment plans and goals
- Case manage clients as needed, ensuring seamless care coordination
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Bachelor's degree in Physical Therapy from an APTA accredited program
- 2-5 years of experience in a rehabilitation or long-term care setting
- Licensed Physical Therapist in MN State Board of Physical Therapy
- BLS Tier - Basic Life Support - Multisource
- Valid Driver's License
- Casual schedule with pay per visit
- Minimum commitment to two weekend day shifts per month
- Flexible schedule with great work-life balance
- Opportunity to pick up more hours
