Job Title: Hospice RN Case Manager

Interim HealthCare of West Michigan is a leading provider of post-acute care services in West Michigan. We are seeking a Registered Nurse to join our Hospice division as a Case Manager.

About the Role

This is a flexible and vital position that requires a RN with active, unrestricted license in the State of Michigan. The ideal candidate will have at least one year of relevant experience as an RN and a strong background in community health and medical-surgical experience.

Responsibilities
  • Complete ongoing assessments of the patient's physical status and psycho-social status
  • Develop and implement a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician
  • Provide education and interventions to support the effective management of the patient's pain and other physical symptoms
  • Ensure that equipment, supplies, drugs, and biologicals are available
  • Make necessary revisions to the IDG plan of care as the patient's/family's status and needs change
  • Complete accurate and timely documentation of patient/family services provided
Requirements
  • RN with active, unrestricted license in the State of Michigan
  • At least one year of relevant experience as an RN
  • One year of management experience (preferred)
  • One year of home health experience (preferred)
  • Current CPR Certification
  • Reliable transportation with a valid state driver's license and current car insurance
