We are seeking a Registered Nurse to join our Hospice division, where you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

As a Hospice RN Case Manager, you will be responsible for providing high-quality patient care, collaborating with our interdisciplinary team, and ensuring that our patients receive the best possible care.

Responsibilities

  • Conduct ongoing assessments of patient physical and psycho-social status
  • Develop and implement individualized plans of care in collaboration with the IDG
  • Provide education and interventions to support effective pain and symptom management
  • Ensure timely and accurate documentation of patient services
  • Participate in IDG meetings and contribute to the development of care plans

Requirements

  • RN with active, unrestricted license in the State of Michigan
  • At least one year of relevant experience as an RN
  • Current CPR Certification
  • Reliable transportation with a valid state driver's license and current car insurance
