About the Role:
As a Home Health Occupational Therapist, you will be responsible for assessing patients' functional status, identifying equipment needs, and developing individualized intervention plans to promote independence and overall well-being.
Your Key Responsibilities:
- Conduct thorough evaluations of patients' physical and cognitive abilities
- Develop and implement effective intervention plans to address patients' needs
- Educate patients and their families on necessary care and activities
- Collaborate with healthcare teams to ensure seamless care delivery
Requirements:
To be successful in this role, you will need:
- An active license to practice as a professional Occupational Therapist in PA
- A minimum of one year of experience in home health or a related field
- Excellent assessment and communication skills

Salary Range:
$80,000 - $110,000 per year, depending on experience
