Job Summary

Hackensack Meridian Health is seeking a highly skilled Ambulatory Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring the safe and effective use of medications.

Key Responsibilities
  • Dispense prescriptions and monitor medication therapy in accordance with regulatory requirements and standards.
  • Review and clarify prescriptions, ensuring accuracy and appropriateness.
  • Monitor patient drug therapy regimens for clinical appropriateness, including potential interactions and adverse reactions.
  • Perform unit and pharmacy inspections to ensure proper maintenance and storage of medications and supplies.
  • Oversee the activities of ancillary personnel, including pharmacy technicians and navigators.
  • Employ principles of safe medication practice and adhere to departmental and organizational policies.
  • Identify patient needs and modify care to meet those needs, communicating effectively with patients and caregivers.
Requirements
  • Graduate of an ACPE accredited school of pharmacy with a minimum of a Bachelor's Degree in Pharmacy.
  • Excellent written and verbal communication skills.
  • Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
Preferred Qualifications
  • A minimum of one year of pharmacy experience.
Licenses and Certifications
  • Registered Pharmacist License.
