Clinical Manager Job Description

As a Clinical Manager at CenterWell Home Health, you will oversee and coordinate all direct care patient services provided by clinical personnel. Your key responsibilities will include planning, implementing, and analyzing clinical operations for a specific location managed. You will also manage caregivers' assignments and ensure delivery of care to all patients served.

Key Responsibilities:

  • Assess and reassess patients, update care plans, and interpret patient needs.
  • Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
  • Coordinate communication between team members, physicians, and caregivers to ensure appropriate care and outcome planning.
  • Participate in sales and marketing initiatives and supervise all clinical employees assigned to a specific location.
  • Assess staff education needs, create and conduct regular staff education as needed.
  • Participate in performance improvement activities and maintain ongoing clinical knowledge through internal and external training programs.

Requirements:

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse and proof of current CPR.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
