Job Summary

The Case Manager RN is responsible for the development, planning, coordination, and administration of functions including activities such as Utilization Review, Discharge Planning, Social Services and Resource Management. This role requires daily review of medical records to determine appropriateness and medical necessity of admissions, continued hospital stay, and use of ancillary services.

Key Responsibilities

• Develop and implement case management plans to ensure optimal patient outcomes
• Conduct daily review of medical records to determine appropriateness and medical necessity of admissions, continued hospital stay, and use of ancillary services
• Coordinate with healthcare teams to ensure seamless patient care
• Identify and address social determinants of health that impact patient outcomes
• Collaborate with community resources to provide patients with necessary support and services

Requirements

• Graduate of an accredited school of professional nursing as evidenced by state licensure
• Diploma or transcripts required upon hire
• A minimum of two years experience in case management or related healthcare setting preferred
• Current Tennessee or Compact State registered nurse required
• BLS certification preferred (AHA or Red Cross)

Work Environment

This is a full-time day shift schedule with a requirement of rotating weekends.
