Palliative Medicine Physician Opportunity

Opportunity Overview
Sutter West Bay Medical Group is seeking a skilled Palliative Medicine Physician to join our Advanced Organ Therapy (AOT) Clinic in San Francisco. As a member of our team, you will have the opportunity to provide high-quality outpatient care to patients with complex medical needs.
Key Responsibilities
- Provide outpatient Palliative Medicine evaluation and follow-up care for patients with liver failure, heart failure, or advanced GI malignancy.
- Facilitate discussions regarding goals of care and advance care planning.
- Assist with complex care coordination and provide support with medical decision-making for patients and their families.
- Provide pain and symptom management support for established AOT patients.
Requirements
- ACGME Fellowship Training in Hospice and Palliative Medicine, Board Certified or Eligible.
- Board Certified in Internal Medicine or Family Medicine.
- At least 2 years of post-fellowship clinical practice in Palliative Medicine, including outpatient experience, strongly preferred.
