**Salary and Benefits:***
We offer a competitive salary of $90,000 per year, as well as a range of benefits including:
- Up to $7,500 sign-on bonus
- Up to $2,500 relocation bonus

**Job Description:**
As an Occupational Therapist at JAG Physical Therapy, your primary responsibility will be to assess patients' strengths and weaknesses, develop effective treatment plans, and maintain patient confidentiality. You will work collaboratively with interdisciplinary teams to deliver high-quality patient care and achieve optimal outcomes.
Key Responsibilities:
- Collections and records patient care data
- Initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with team members
- Daily billing and following department guidelines for productivity
- Preparation for evaluations according to department policies/procedures and standards of practice
- Selecting and administering tests and measures
- Synthesizing evaluation results to formulate diagnosis, goals, prognosis, and plan of care
- Establishing treatment plans based on evaluation results, goals, and best practices
- Collaborating with interdisciplinary teams, patients, and patient support systems
**Required Skills and Qualifications:**
To be successful in this role, you will need to possess the following skills and qualifications:
- Bachelor's or Master's degree in Occupational Therapy required, Doctorate preferred
- Licensed Occupational Therapist
- Completed state-regulated CEU requirements in the appropriate state (NY, NJ)
- Valid CPR license
- Experience/knowledge with EMR software preferred
- Excellent oral and written communication skills
- Strong clinical decision-making skills
- At least one year of professional OT experience
