Job Summary

CVS Health is seeking a highly skilled and compassionate Case Management Coordinator to join our team in Ohio. As a key member of our OhioRISE program, you will play a critical role in providing comprehensive and highly coordinated behavioral health services for children with serious/complex behavioral health needs.

Key Responsibilities
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans that address the unique needs of each child and family.
  • Conduct comprehensive assessments and evaluations to identify high-risk factors and service needs that may impact member outcomes.
  • Coordinate and implement assigned care plan activities, monitoring progress and making adjustments as needed.
  • Work closely with members' Child and Family Teams to ensure seamless care coordination and support.
  • Identify and escalate quality of care issues through established channels, utilizing negotiation skills to secure appropriate options and services.
  • Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices.
Requirements
  • 2+ years of experience in behavioral health, social services, or human services.
  • 2+ years of experience with personal computers, keyboard, and multi-system navigation, and MS Office Suite applications.
  • 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field, providing community-based services to children and youth, and their family/caregivers.
  • Willing and able to travel within the assigned region up to 50% of the time.
  • Reliable transportation required.
Preferred Qualifications
  • Case management and discharge planning experience.
  • Managed Care experience.
  • Medicaid experience.
