Description

UCFS is seeking a per-diem Registered Nurse who will utilize critical thinking skills to interpret complex information in a multidisciplinary setting to guide patients and families to health and wellbeing across the continuum of care in an outpatient setting.

Responsibilities:

  • Provide nursing support for a large outpatient practice, including nursing consultation, assessment, patient follow-up, and patient surveillance of health statuses and outcomes.
  • Assist with physical exams, complete required screenings, and administer treatments, immunizations, and medications to clients.
  • Provide client services, educate clients/parents as needed, assist with prescription needs/refills, coordinate services with/for clients, and act as a client advocate to pursue financial assistance and initiate referrals to community-based organizations as needed.
  • Respond to calls from patients, pharmacies, home care agencies, and family members, collaborate with school nurses, DCF, and other providers as needed, and complete paperwork needed by external organizations.
  • Work collaboratively with behavioral health and dental departments to facilitate integrated care for patients.

Requirements:

  • Registered Nurse licensed to practice in the State of Connecticut with at least 3-4 years of experience (primary care experience preferred, including pediatrics and internal medicine)
  • CPR certified
  • Strong communication and interpersonal skills
  • Prior experience utilizing an electronic health record system is a strong plus
  • All employees at UCFS are required to be vaccinated against COVID-19
