We are seeking a highly motivated and empathetic Care Coordinator: Peer Specialist to join our team. This exciting opportunity allows you to leverage your lived experience to make a meaningful difference in the lives of adults with mental health disorders.

Key Responsibilities

  • Provide comprehensive care coordination and rehabilitation services to individuals in their natural settings.
  • Work closely with clients and their families to provide resources, guidance, and support.
  • Liaise with community resources and services to ensure seamless support.
  • Maintain accurate records and documentation.
  • Collaborate with multidisciplinary teams to achieve optimal outcomes.

Qualifications

  • Lived experience as a consumer in the Adult Mental Health; Alcohol and Other Drug; and/or Health, Housing, and Homeless service domains.
  • S.P.I.R.I.T. or P.E.T. Graduate, Certified NAMI Peer to Peer Instructor, or equivalent knowledge and training required.
  • AA, BA, or Masters in Social Work, Counseling, Psychology, or related field with several years professional experience in behavioral health preferred but not required.
  • Experience being a consumer advocate preferred.
  • Speaking, writing, and reading fluency in Spanish is preferred but not required.

What We Offer

We offer a competitive salary range of $21.00/hour to $28.69/hour, depending on qualifications and experience. Our comprehensive benefits package includes medical, dental, and vision coverage, as well as retirement savings options.
