About the Role

AdvisaCare is seeking a skilled professional to fill the position of Home Health Intake Coordinator. As a key member of our team, you will play a vital role in ensuring seamless patient onboarding and care coordination.

Key Responsibilities
  • Verify patient insurance coverage and obtain necessary authorizations
  • Document patient information accurately and efficiently
  • Collaborate with discharge planners, referral sources, and insurance providers to ensure smooth patient placement and service initiation
  • Coordinate scheduling of clinical staff and ensure timely start of care
Requirements
  • High school diploma or equivalent
  • Knowledge of insurance verification and authorization processes
  • Experience with electronic medical records (EMR) systems, preferably Wellsky/Kinnser
  • Strong organizational and multitasking skills
  • Excellent verbal and written communication skills
