We are seeking a skilled and compassionate Health Home Care Coordinator to join our team at Trinity Health. As a key member of our Care Management Team, you will play a vital role in coordinating and collaborating with community providers to ensure positive outcomes for our members.

Key Responsibilities
  • Develop and maintain professional relationships with members and community providers to ensure coordination and collaboration of services.
  • Conduct comprehensive assessments to identify members' medical and behavioral health needs, substance abuse, activities of daily living, socio-economic and housing status, and social determinants of health.
  • Develop and implement person-centered care plans that coordinate and integrate a comprehensive array of services to support members' optimal wellness and recovery.
  • Collaborate with an interdisciplinary care team to ensure seamless coordination of services and effective communication with members and providers.
  • Provide education and support to members and their families to promote self-management of their healthcare needs.
  • Advocate for members' needs and rights, and facilitate access to community resources and services.
Requirements
  • Bachelor's Degree in Human Services or related field.
  • Two years or more experience working with individuals with chronic medical conditions and behavioral health issues.
  • Excellent planning, organizational, and communication skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.
