Job Title: Service and Support Associate

Join the Allen County Board of Developmental Disabilities as a Service and Support Associate, where you will play a vital role in providing exceptional support to individuals with developmental disabilities.

Job Summary:

We are seeking a highly motivated and compassionate individual to serve as the primary point of coordination for individuals receiving Service and Support Administration (SSA) services. As a Service and Support Associate, you will be responsible for assessment, planning, coordinating, and monitoring of all services, ensuring that individuals receive the support they need to live fulfilling lives.

Key Responsibilities:
  • Facilitate the person-centered planning process to identify and address individual choice, preferences, strengths, and needs for all services.
  • Assess individuals' need for service utilizing formal and informal assessments.
  • Develop and revise Individual Service Plans (ISPs) to ensure effective and meaningful outcomes.
  • Work cooperatively with individuals and on behalf of individuals to coordinate and acquire needed services.
  • Link individuals to appropriate community resources and assist individuals in the provider selection process as required.
  • Maintain effective and ongoing contact/communication with individuals, families, guardians, and significant others, as applicable.
  • Continuously review the delivery of services, supports, and training provided by the Board, other agencies/providers, and persons important in the life of individuals served.
  • Ensure services are effective and in compliance with local, state, and federal regulations.
  • Develop staffing patterns and service budgets to maximize services while maintaining costs.
  • Monitor utilization of authorized services and maintain proficient knowledge of assessment tools and software programs for adequate waiver service planning and budgeting.
Requirements:
  • Bachelor's degree.
  • Over one through three years of related experience preferred.
  • Experience with Medicaid/Medicare Waiver programs strongly preferred.
  • Experience with standard office equipment, including computers with Microsoft Office and similar software for advanced computer use.
  • Have or be capable of obtaining Service and Support Administration certification.
  • Must have a valid driver's license with a record that is acceptable by the agency's insurance carrier as verified by an annual driver's abstract.
  • Must provide proof of liability insurance on personal vehicles.
  • First Aid and CPR certification.
Working Conditions:

This position requires a flexible schedule, including the possibility of telework. The employee may be required to work in a fast-paced environment and make decisions in a timely manner.
