- Administrative Support: Provide administrative assistance to healthcare providers, including data entry, record-keeping, and other tasks as assigned.
- Program Evaluation and Planning: Participate in program effectiveness analysis and planning to ensure high-quality care and services.
- Member Visits and Rounding: Conduct regular visits and rounding to assess member needs and provide support.
- Communication and Education: Communicate with members, caregivers, and healthcare providers to educate them on care plans, services, and resources.
- Data Collection and Entry: Collect and enter data into electronic medical records, including vital signs, functional status, and other relevant information.
- Care Coordination Activities: Coordinate care activities within the healthcare setting to ensure member needs are met, including scheduling appointments, arranging transportation, and procuring durable medical equipment.
- Current Unrestricted RN License: Possess a current, unrestricted Registered Nurse license in Oregon.
- Experience and Education: Hold a minimum of 2 years of RN experience and 1 year of case management experience, with a Bachelor of Science in Nursing (BSN) degree preferred.
- Technical Skills: Proficient in using electronic medical records and Microsoft Office software, with the ability to navigate a Windows environment.
- Travel and Availability: Possess a valid driver's license and be able to travel locally 75% of the time, with a flexible schedule that includes up to 2 days per week.
