About the Role

We are seeking a highly skilled MDS Coordinator RN/LVN to join our team at Skyline Nursing Center. As a key member of our healthcare team, you will play a vital role in ensuring the timely and accurate completion of the RAI process and care management process.

Key Responsibilities
  • Collaborate with the Interdisciplinary Team to assess the needs of residents and provide interdisciplinary schedules for MDS assessments and care plan reviews.
  • Ensure that the Interdisciplinary team makes decisions for completing or not completing additional MDS assessments based on clinical criteria.
  • Assist with coordination and management of the daily stand-up meeting to review resident care and set assessment reference dates.
  • Comply with federal and state regulations regarding completion and coordination of the RAI process.
  • Monitor MDS and care plan documentation for all residents and ensure documentation is present in the medical record to support MDS coding.
  • Maintain current MDS status of assigned residents according to state and federal guidelines.
  • Maintain frequent and accurate data entry of resident information into computerized MDS programs.
  • Complete accurate coding of the MDS with information obtained via medical record review, observation, and interview with facility staff, residents, and family members.
  • Attend interdisciplinary team meetings, quality assurance meetings, and other meetings to gather information, communicate changes, and maintain and update records.
  • Assist the Director of Nursing or designee with identifying significant changes, physician orders, and verbal reports to ensure the MDS and care plan are reflective of those changes.
  • Prepare scheduling, notice of resident care planning conferences, and assist the Director of Nursing in communicating outcomes and problems to responsible staff, residents, and/or responsible parties.
  • Attend weekly Medicare, Medicaid, and Managed Care meetings as required.
  • Correct and ensure completion of final MDS and submit resident assessment data to the appropriate State and Federal government agencies.
  • Assign, assist, and instruct staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required), and clinical computer system in relation to these processes.
  • Maintain confidentiality of necessary information.
Requirements
  • Graduate of an approved RN/LVN program and licensed in the state of practice, required.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
  • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
  • Knowledge of the care planning process.
  • Experience with MDS 3.0, preferred.
